痛風(因血中尿酸濃度異常過高所致.俗名帝王病)

一.前言:                                                                                                                                                                     所謂[痛風],即是一種文明病,痛起來驚天動地,但來得快也去得也快,有如一陣風,故有[ 痛風]之稱.痛風是指尿酸結晶進入關節腔中,所引發的急性發炎,急性發作通常會侵犯下肢單一關節,或關節旁組織,尤其大腳趾的蹠趾關節,踝關節或膝關節,發炎的關節會呈現[紅,腫,熱,痛]的反應,部分病例還會合併發燒,故容易被誤診,以為是蜂窩性組織炎或細菌性關節炎.                                                           

[痛風]是體內尿酸產生過多,或排泄不良所引起的,在中醫屬於[痹症]的範圍, 常見於營養太好的人,素食者,或常吃瘦肉者,突發在半夜或清晨,部位上以腳的大拇 趾較常見,其次見於踝,手,腕,膝,肘,以[紅,腫,熱,痛,關節不靈活]為主要特徵.有時 會合併輕微發燒,待關節炎消退後可能會有一些脫皮現象.誘發痛風的因素包括:喝酒 .過度攝取高普林食物,運動,受傷,手術,出血,感染,放射線治療,抗結核菌藥,阿司匹 靈,及一些降血壓藥(利尿劑.Thiazide).                                                                          

臨床上對高尿酸血症的定義:是男性的空腹血清尿酸濃度大於7mg/dl,停經前女性大於6mg/dl,大多數患者是因為腎臟排泄尿酸減少所致,少數是因尿酸產生過多所引起. 痛風的發生和高尿酸血症具有密切關係,即血中尿酸越高,越容易發生痛風.尿酸是普林代謝的最終產物,因人類缺乏分解尿酸的酵素因此會存在高尿酸血症的問題.                                                                                                                                                        

二.好發部位:                                                                                                                                                          早期發作時即使不吃藥,過了3-10天也會自動不痛,臨床上以單一關節炎最常見,但發 作越頻繁,可能轉為多關節炎或是疼痛時間因此而拉長.第一個腳拇指為常侵犯處.其 他像腳踝,腳跟,腳背,膝,腕,肘,手指也都可能發生,但以下肢為最常見處.                                                                                                     

三.年齡層與性別:                                                                                                                                                  根據臨床研究,目前在台灣成年男性,痛風的發生率高達3.3%,男性比女人易發生此病,可能跟應酬與喝酒多有關係.男性以40-50歲 為好發年齡,而女性以60歲以後為好發年齡,所以停經前女性及30歲前男性(非胖者), 少有痛風的發生,但隨著國人飲食習慣的改變,罹患痛風的年齡有日漸下滑的趨勢.                                                                           

四.後遺症:                                                                                                                                                               痛風除了會伴隨痛風性關節炎,形成痛風石外,其他還可能引起痛風性腎病變與因尿 酸引起的尿路結石等,通常痛風第一次發作後.一年內再發率為60%.只有約7%的患者 一輩子只發作一次.        

患者若因尿酸而產生尿路結石,其發生率高達10-20%,造成的影響是腎臟可能因此受損,嚴重者甚至必須終身洗腎 .痛風如未受到良好的控制,長年下來,將會因尿酸鹽沉積而在關節,皮下組織,耳輪及內臟形成痛風石.  

研究指出:平均每升高1mg/dl 血尿酸值,對 1個有高尿酸又合併高血壓的人,將增加32%得到心血管疾病的機率,如合併冠心病者將提高26% 的死亡率,可見尿酸過高除了會引起痛風外還會引起腎臟病,心血管疾病,平時必須特別注意飲食禁忌 .                                                                           

五.禁忌----以節制食量,避免肥胖,不吃有殼的東西(包括動植物)或植物的種子,動物的 生殖細胞和內臟及瘦肉.                                                                                                                                                                              1.禁止喝酒,尤其啤酒絕對不能喝.因酒精在體內代謝產生乳酸,會使血中的乳酸濃度升 高,導致腎臟的尿酸排泄減少,所以大量飲酒後會誘發痛風的發作.多樣的酒類中以啤 酒的嘌呤含量最高,而白蘭地和威士忌則幾乎不含,因此痛風的人絕對不能喝啤酒,非 不得已則改喝一點點白蘭地和威士忌.                                                                                                                                                                             2.瘦肉少吃.(肥胖的人,肥肉也不能吃).                                                                                                               3.動物的內臟或生殖細胞.如:蛋,魚卵,烏魚子,腦髓,睪丸,腸子,心,肝,肫.腰子.脆腸.                                                                                                                                                                             4.有殼沒骨頭的海鮮,如:青蚵,文蛤,花枝,尤魚,鱆魚,透抽,小管,蝦蟹,龍蝦,干貝,海膽                                                                                                                                                                               5.魚類包括:加納魚.白鯧魚.白帶魚.沙丁魚.鯊魚.海鰻.秋刀魚.皮刀魚.吳郭魚.鰱魚. 虱目魚.烏魚.吻仔魚.四破魚.                                                                                                                                                                              6.植物的種子.如:瓜子,紅豆,綠豆,落花生,蠶豆,栗子,蓮子,毛豆,黃豆,杏仁.                                                                                                                                                                             7.黃豆(大豆)做的食品.如:豆漿,豆腐,豆干,豆花,素肉,素雞.                                                                                                                                                                             8.菇類不能吃.如:香菇,磨菇,草菇,金菇,菘蕈,猴頭菇.                                                                                                                                                                             9.不喝雞精,養樂多,高湯或火鍋湯,肉湯.酵母粉.其他如:豆苗,紫菜,蘆筍,竹筍尖,白花椰菜, 菠菜也要少吃為妙.                                                                                                                                                                        10.某些藥物會造成尿酸升高,如:利尿劑,肺結核藥,阿斯匹靈.服用這類藥物要小心.                                        

六.多吃水果和葉菜類(但綠豆芽,黃豆芽或發芽豆類要把前面的葉子弄掉,才能食用).                               

七.宜多喝水,少熬夜,不要太勞累.勿作激烈運動,因肌肉易缺氧,使體內尿酸增加.                                         

八.天氣冷要保暖,因溫度冷會讓尿酸晶體沉澱,引起痛風發作.                                                                       

九.不宜快速減輕體重,以避免身體組織急速分解,誘發痛風.                                                                          

十.要有決心耐性控制飲食,不該吃的就不要吃或節制少吃.預防比治療重要,飲食控制比 吃藥重要,能少吃藥而身體健康,總比有些醫生叫你天天吃藥還好,誤把肚子當作藥庫                                                               

十一.食療:                                                                                                                                                                 1.櫻桃茶飲------櫻桃對於痛風的人是非常有益的水果,生吃,打汁,罐頭的皆有用.                                                                                                                                                                              1)材料:櫻桃原汁15cc,白蘭地3cc,紅茶1包,開水200cc,糖適量.                                                                                                                                                                             2)作法:紅茶包用200cc開水泡3分鐘,取出茶包,加糖,櫻桃原汁,白蘭地調勻.                                                                                                                                                                             3)功效:櫻桃含花色素(anthocyanidin)和前花色素(proanthocyanidin),能降低尿酸, 又能穩定關節的膠原質結構,含有韖花酸(Eiiagic Acid),有抗氧化作用,還含 有一種鎮靜的化學物質be   nzodiazepines,使緊張的情緒得到紓解,不僅如此, 它所含的柳酸鹽也具有抗發炎和鎮痛的作用.                                                                                                                                                                        

    2.哈蜜瓜草莓汁------                                                                                                                                                                                       1)材料:哈蜜瓜200克,草莓200克,開水適量.                                                                                                     2)作法:哈蜜瓜洗淨,去皮切小塊,草莓洗淨去蒂,將所有材料放入果汁機中攪打成汁.                                                                                                                                                                              3)功效:痛風發作,關節急性發炎的現象,會使體內的自 由基濃度升高,此時多攝取抗氧 化的物質能抑制自由基濃度,預防關節受到第二次傷害.哈蜜瓜所含的B-胡蘿 蔔素和草莓所含的韖花酸,正是抗氧化劑,能避免關節受到自由基的攻擊.                                                                                                                                                                          

  3.紫蘇梅子汁------                                                                                                                                                                                   1)材料:紫蘇梅濃汁10cc,開水200cc.                                                                                                                 2)作法:紫蘇梅濃汁以開水調勻.                                                                                                                            3)功效:紫蘇葉中的迷迭香酸有抗炎作用,薄荷醇有局部麻醉的效果,梅子雖酸,經過消 化之後會變成鹼性物質,  可以維持血中尿酸濃度,避免大幅升高,梅子所含的柳 酸鹽具有抗發炎和鎮痛的作用.                                                                                                                                                                         

   4.菊花甘草茶------                                                                                                                                                                                   1)材料:菊花5朵,甘草10片,水適量.                                                                                                                     2)作法:將所有材料放入鍋中,以中火熬煮5分鐘,濾出湯汁.                                                                                                                                                                             3)功效:菊花,含有parthenolide能抑制前列腺素-II和白血球,減輕關節炎的疼痛和發 炎現象.甘草,含有甘草次酸能抑制中性白血球的代謝,降低活性氧原子的形成 而具消炎效果.                                                                                                                                                                         

   5.檸檬原汁-----將檸檬洗淨壓汁,可原汁喝,或加少許蜂蜜,果糖,或橄欖油2-3滴,或加 開水稀釋,一天喝250cc多多益善.檸檬含有豐富的檸檬酸(citric acid),本身 屬於酸性的,但進到胃裡,經吸收後會變成鹼性物質,可促進乳酸lactate代謝, (乳酸是身體疲勞,運動後或下班工作後肌肉的酸痛之元凶)排除乳酸廢物,有 抗氧化作用,降低尿酸的濃度.又因含大量的維他命C,除了抗氧化,抗衰老,抗 癌外,又可養顏美白,使人皮膚嬌嫩,青春又美麗.                                                                                                                                               

十二.總結: 痛風是一種代謝障礙的疾病,與吃太好,太營養有絕對關係,病患本身要確認此病的嚴 重性,而且此病只能控制不能根治.初次發病後,就會有再發的機會,所以一定要有耐 心,毅力,意志力要強,一定要忌口,去控制飲食,不該吃的就不要吃或少吃.不要以為 這樣,那人生還有什麼意義,勿可憐自己,為何吃得這麼不好,以為是在虐待自己,和自 己過意不去,人生感覺乏味.要體認---[健康就是財富,沒有了健康,則一切歸於零.]. ----------------------------------------------------------------------------
